How to register

brookhouseTo register come into the surgery and we will ask you to complete some forms:

  • A health questionnaire to help us to look after your needs as fully as possible.
  • A registration form

Both forms can be downloaded from the above "Registering Online" tab.

Bring your medical card from your previous GP if you have one, but don’t worry - it is not necessary for registration.

We will also ask for evidence of identity and of residence in our area - a current utility bill plus photo ID would be ideal.

You will be allocated a Doctor and you will have the right to express a preference to see your allocated Doctor.  However, if this is not possible, you will be offered an appointment to see another healthcare professional.

If you are already taking medication, or have medical needs of any kind, we will invite you for an early appointment with either the doctor or nurse.

Medical treatment is available from the date of registration. Please contact reception for further information.
