Complaints

If you have a complaint please contact the the Practice Manager who will deal with your concern through the practice Complaints Procedure.  You can do this in writing via letter, via our email address hiowicb-hsi.brookhousesurgery@nhs.net or via a phone call to the Practice Manager.  As you will need to be identified via your patient record, it would be helpful to indicate your name, date of birth and address in your communication.  If you are complaining on behalf of someone else, we will need their written consent to be able to address the issue raised

Compliments

We welcome any compliments you would like to make in relation to any of our staff and the service we provide.  You can do this in writing via written correspondence, via our email address hiowicb-hsi.brookhousesurgery@nhs.net or via phone call directly to the Practice Manager.

Comments

We have a suggestion box in our waiting room area, please do complete a form if you feel you have a comment you would like to make to help us improve the surgery or our service to you.
